Information for patients

As of May 2024, private clinics previously situated within the Sessional Suites at St John of God Midland Public and Private Hospitals will be relocated to our new practice located within the Railway Workshop building at 6 Centennial Place Midland. Below is some helpful information to ensure your appointment runs as smoothly as possible.

  • Please arrive to your appointment on time.
  • Prior to your appointment, write down any symptoms you are experiencing. Please include any that may seem unrelated to the reason for which you scheduled the appointment.
  • Write down key personal information which you may need to fill out on a registration/surgery booking form – for example your Medicare and private health insurance details.
  • Please bring:
    • Your Medicare card and heath fund details
    •  A list of all your medications including prescription medications and over the counter drugs (including vitamins and supplements).
    • All imaging reports, scans and blood results
  • You are more than welcome to bring a family member or friend along. Your support member may remember things you have missed or forgotten.
  • Write down questions to ask your surgeon. There is no such thing as a silly question!
  • If your problem is not acute or an emergency, please consult your General Practitioner.
  • If you are in a medical emergency, please present yourself to the emergency department at your nearest hospital.
  • If you have a referral to see a specialist surgeon in our team, please contact us on the details provided.
  • If you do not have a referral, however are keen for more information about our services, please call/email the respective clinic.
  • We only offer surgical services to people over 16 years of age.
  • We provide no gap surgery to our patients. Exclusions may apply for breast reduction surgery.
